Pulmonary circulation and ventilation.

چکیده

The pulmonary circuit receives the whole of the cardiac output, and the flow through it is governed by events in the systemic circulation. Its function is gaseous exchange, and the low resistance and high capacitance of the circuit is subservient to this function. Local flow is affected by intra-alveolar oxygen tensions and regional flow by passive hydrostatic effects due to gravity. The distribution of ventilation is also affected by gravity owing to the effect of the latter on regional compliance. When the cardiac output falls, as it does in most methods of induced hypotension, there is an accompanying increase in physiological dead space owing to an increase in the alveolar component.

Regional ventilation in the differential diagnosis of pulmonary embolism.

In the diagnosis of pulmonary embolism by lung scanning, clinical errors of interpretation may arise. Diseases that affect the distribution of pulmonary blood flow, such as pulmonary emphysema and bronchial asthma, may be confused with pulmonary embolism. With the addition of ventilation studies with 133xenon to the perfusion scans, distinct differences appear between patients vith emboli and t...

Postoperative Pulmonary Dysfunction and Mechanical Ventilation in Cardiac Surgery

Postoperative pulmonary dysfunction (PPD) is a frequent and significant complication after cardiac surgery. It contributes to morbidity and mortality and increases hospitalization stay and its associated costs. Its pathogenesis is not clear but it seems to be related to the development of a systemic inflammatory response with a subsequent pulmonary inflammation. Many factors have been described...

Changes in the Pulmonary Circulation after Bronchial Occlusion in Anesthetized Dogs

The effects of bronchial occlusion on the pulmonary circulation were studied in anesthetized cats and dogs. Immediately after occlusion, blood flow fell rapidly at normal pulmonary arterial pressure (—71% cats, —54% dogs), and in constant flow perfusion experiments, perfusion pressure rose (+50% cats, +28% dogs). These resistance changes were reversed by vasodilator drugs, alkali, or perfusion ...

Reconstruction of bilateral branch pulmonary artery stenosis caused by Takayasu's aortitis.

A 63 year-old female presented with dyspnea on exertion. Her chest X-ray showed cardiomegaly, and right ventricular overload and tricuspid regurgitation were detected. Her pulmonary ventilation and blood flow scintigraphy findings were suspicious of pulmonary vascular disease; the diagnosis was pulmonary hypertension and bilateral branch pulmonary artery stenosis. After the inflammation settled...

Hypoxia is/is not the optimal means of reducing pulmonary blood flow in the preoperative single ventricle heart.

COMMENT ON POINT:COUNTERPOINT To the Editor: Optimal may depend on the circumstances and both points have utility (1, 3). We found carbon dioxide manipulation more useful and less dangerous than alveolar hypoxia in acute transitional settings when mechanical controlled ventilation is required in unrepaired ductal or shuntdependant pulmonary blood flow (2). The technical aspects of transitioning...

0681. Effects of veno-venous co2 removal therapy on pulmonary circulation in an ARDS model

Introduction Acute respiratory distress syndrome (ARDS) is responsible for injuries to the alveolar epithelium and microvascular endothelium resulting in hypoxemia, decreased pulmonary compliance and increased pulmonary vascular resistance. Beneficial effects resulting from protective lung ventilation are counterbalanced by deleterious hemodynamic effects. Indeed, hypercapnia resulting from ven...
